We have now been open since October 1st, 2020. We are thankful for the opportunity to provide high-value, affordable service for the people of the surrounding community. Since you are on this page, you have an interest in “The way primary care should be”, but still have questions about the model and the practice. The pre-registration will allow us to contact you and set up a consultation (by phone or in person visit) to answer any questions you may have. This is not enrollment into the practice, but a way of getting to know each other to see if this model of care works well for you. If it does, then we can move forward with enrollment. There is no obligation by pre-registering and the consultation is free.
